What we take off your plate.

When you refer a family to BlossomCare, we manage the entire enrollment process from start to finish. That includes:

  • Coordinating and supporting communication with your Adult Services Worker and medical staff
  • Completing the Home Help Application
  • Completing required documentation
  • Preparing the family for the in-home assessment
  • Guide through CHAMPS enrollment and background check
  • Setting up payroll and direct deposit
  • Maintaining all ongoing compliance documentation

You make the referral. We handle everything after that.

How long does enrollment take?

Most families complete the enrollment process within four to eight weeks from first contact to first paid hours. We move as fast as the process allows, and we follow up on every outstanding item so nothing stalls unnecessarily.
