For the Client
Steps One to Four- 01
Step One: The Medical Needs Form
Your loved one's primary care physician will complete a Medical Needs form documenting the conditions and daily living limitations that qualify them for the program. Bring it to your next appointment, or we can coordinate directly with the office. Once it is signed, we collect it from you.

Step Two: The Home Help Application
Sections one and two of the application require your signature. The remaining sections are administrative, we walk you through each one personally. Most families complete this in a single sitting.

Step Three: The In-Home Assessment
The Adult Services Worker visits your home in person to observe daily living needs and determine the approved hours of care. This visit is conversational, not clinical. They are there to understand the household, not to evaluate it. We help you prepare for what they will ask.

Step Four: Care Becomes Compensated
Once hours are approved, the caregiving that has been happening informally becomes a structured, paid role. BlossomCare manages payroll, tax compliance, and all ongoing documentation with our insurers. You provide the care. We hold the structure around it.

Step One- 01
Step One: Meet the Baseline
You must be at least 18 years old with no disqualifying criminal history. This is the starting line for every caregiver we enroll.

Step Two: Pass the Background Check
Complete and pass the background check through CHAMPS, the state's provider enrollment system. The official step-by-step instructions are linked below.

Step Three: Complete the Required Training
Finish the training required of every enrolled caregiver. It is straightforward, and we make sure you know exactly what to complete and in what order.

Step Four: Start Getting Paid
Once your background check clears and training is done, the care you are already providing becomes a structured, paid role. We handle payroll, tax compliance, and ongoing documentation from here.

Common Care Needs
Higher care needs often mean more paid hours.
The amount of approved care is based on an individual's needs and ability to safely complete daily activities. In many cases, those with more significant care needs may qualify for additional paid caregiving hours.
Examples of conditions that often require daily, hands-on support include:

Eligibility and approved hours are determined by our insurers based on an individual's specific care needs and circumstances.
